Disenrolling Before
Completing the Initial Enrollment Period for
Active Duty Family Members
The following table describes valid reasons for
active duty family members disenrolling (canceling
coverage) from the TDP before the end of the
mandatory 12-month initial enrollment period:
Situation
Description
Loss of eligibility
Sponsor or family member loses
eligibility for the TDP due to death, divorce,
marriage, age limit of a child, or end of entitlement.
Sponsor and family are relocated to the OCONUS
service area
Sponsor may elect to disenroll
his or her family members from the TDP within
90 calendar days of the relocation. The date of
the relocation must be included on the disenrollment
request. The disenrollment will be processed based
on the date the TDP Enrollment/Change Form is
received.
Active duty sponsor receives Permanent Change
of Station orders
If an active duty sponsor transfers
with TDP-enrolled family members to a
duty station where space-available dental
care is available at the local uniformed services
dental treatment facility (DTF), the active
duty member may elect to disenroll his or her
family member(s) from the TDP within
90 calendar days of the transfer. The date
of the transfer must be included on the
disenrollment request. The disenrollment will
be processed based on the date the TDP
Enrollment/Change Form is received.
