Dental Implant Cost-Shares
For implant services received OCONUS, you are responsible to pay
a 50-percent cost-share.
If you are Command Sponsored, the Government will pay any difference
between the remaining 50 percent and the $1,200 annual maximum.
The Government will also pay any difference between the dentist's
charge and United Concordia's allowance (except for National
Guard/Reserve, IRR family members and IRR [other than Special
Mobilization Category] enrollees).
For treatment plans involving implants, United Concordia recommends
that a predetermination be obtained before implant services are rendered.
A predetermination,
ensures that both you and your dentist will know before you
receive treatment if the proposed service(s) will be covered
and what the anticipated amount of payment will be from
United Concordia.
